Affordable Health Care Plans

Camelback Family Health Care provides affordable health care by offering special affordable cash pricing plans to help non-insured patients. These plans help offset the high cost associated with receiving quality health care and should only be considered if no insurance options exist or if you have an indemnity (catastrophic) health plan. Our affordable cash pricing plans are specific for primary health care delivered by Camelback Family Health Care. These cash pay plans do not include referrals to specialists, laboratory fees charged by LabCorp or Sonora Quest, or pharmacy costs.

PRN PLAN HIGHLIGHTS (PLEASE REFER TO PRICING SHEET)

  • New patient visit- $85 for 1 to 2 problems; $105 for complex problems
  • Established Acute visit- $65 for 1 to 2 problems; $85 for complex problems
  • Well Woman Examination (includes pap smear, laboratory cost is extra)- $125
  • Sports Physical- $75
  • Preventive Physical (evaluation only)- $105
  • Well Child Exam(evaluation only)- $105

PRN Plan Pricing Sheet

COMPREHENSIVE PREVENTIVE PLAN

Our Comprehensive Preventive Plan has a $350 enrollment fee which includes:

  • One initial preventive examination and labs (CBC and Comprehensive Panel)
  • Three follow up visits for any reason. After three visits patient pays $45
  • Flu Vaccine
  • Discounted Pharmacy through local independent partner
  • 15% discount off vaccines and procedures at Camelback Family Health Care
  • Priority scheduling within 24 hours of calling
  • Patient portal access

Plan Exclusions:

  • Additional laboratory fees from LabCorp (our preferred partner). These fees are discounted
  • X-rays, MRI’s, Ultrasounds. Discounted price through our preferred partner
  • Specialist fees
  • Medical equipments, supplies, and other necessary items
  • FMLA paper work or other paper work. $35 charge
  • Hospitalizations
  • Emergency Room or Urgent Care visits
  • EMS, Ambulance, or public transportation services
